The report said Pate was westbound when his truck crested a hill; crossed the center of Northeast 100th Street and began to skid; crossed back over the center of the road, and went off the right side. The driver over-corrected with the truck going across the road while rotating 180 degrees, then went off the left side of the road where it overturned, struck a fence, and came to rest on its top on the shoulder of the road.

The truck was demolished. The report said Kenneth Pate was using a safety device.
